REACH US Initiative (Racial and Ethnic Approaches to Community Health) |

Community Health Councils is one of only 42 national demonstration projects and two community-based organizations funded by the Centers for Disease Control and Prevention (CDC) as part of REACH 2010. REACH 2010 is managed by the CDC to support Healthy People 2010--the nation's health promotion and disease prevention initiative. The goals for Healthy People 2010 are twofold:
To increase life expectancy while improving the quality of life for individuals of all ages To eliminate health disparities experienced by individuals because of race, ethnicity, gender, education, income, disability, locality and sexual orientation.The primary objective of REACH 2010 is to help communities mobilize resources to support programs that eliminate the health disparities experienced by racial and ethnic minorities. REACH 2010 health priorities include infant mortality; breast and cervical cancer screening and management; cardiovascular disease; diabetes; child and adult immunization; and HIV/AIDS. Other California grantees include:
California Black Health Network: CVD and Diabetes (AA) San Francisco Department of Public Health: Infant mortality (AA) Harbor-UCLA Research & Education - Immunization (AA / Latinos) University of California, San Francisco: Breast & Cervical Cancer (API).
